(Because of a conflict with a TV Ad campaign using a similar Toll Free number each faxed order was costing us an average of $2 each, due to wrong number calls. Since most of these calls would cost you less than $.10 we we have discontinued our Toll Free Fax line. )

Complete the "Copy & Paste Order Form"; found at the links below.
When complete, click anywhere outside an entry field
Next press "Ctrl-A"; or on menu, select EDIT>SELECT ALL
With all text highlighted press "Ctrl-P"; or on menu, select File>Print
Select the FAX as the printer, if faxing from your computer, or print a physical copy and fax from a fax machine.

NOTICE: Do not send entire Credit Card Number, only the last four digits. If we do not have this card on file, call in the number!
